The Performed Procedure Step option was installed and it caused this issue
Symptom
When Exams are networked to the PACS, each Series shows as a separate Exam For example, Exam 123456 has 7 series in it. When networked to the PACS, instead of having 7 series in the one exam, shows as 7 separate exams, all with the same exam number.
