Signa HD 1.5T NV Coil Intermittent Driver Faults Effectivity: All Signa Excite HD (12.x) systems with the 1.5T HD NV Coil by Medrad (M3087JJ, 5117092-2). This coil can be identified as the only 1.5T neurovascular coil with the new HD system connector. Problem Description: The Medrad HD NV Coil may demonstrate intermittent open circuit driver faults (error code 2252301) during scanning. Investigation has indicated that this failure is caused by a combination of system DC fault specifications and gradient-induced signals on the coil DC lines. The frequency of failure will vary by site (many sites showing no failures at all) due to several site-varying parameters that impact bias currents (cable length, etc.) Corrective Action: Install 12.0 M4 Patch 10 (patch_12.0_M4_10-PA-2.i386.rpm). This patch modifies the bias voltages and open circuit fault values to prevent driver faults. After patch installation all HD NV coil names must be removed from the site list and added back in using the config file manager. This must be done for the changes provided by the patch to take effect. See the readme note included with the patch for details. Thank you! An open circuit fault occurs when the actual value is less than the pre-defined limit. Check system cables, connectors, coils and configurations. Re-seat all coil connectors. The mask of all failed channels indicates additional channels with the same fault - convert the hex number to a bitmap to decipher the mask, with channel 1 indicated by the least significant bit and channel 16 indicated by the most significant bit. For example, a mask of 0x3c converts to 0011 1100, meaning channels 3, 4, 5, and 6 all experienced the same failure.
