ICN Issues in the Field PROBLEM : A few issues have been reported in the field regarding the functionality of the ICNs. Please read below, as the effectivity for each issue may be different. SOLUTION : This service note is intended to highlight the following issues and provide a valid workaround: 1. INFINIBAND FIRMWARE ISSUE – Applicable to 14M5A & 15.0 M3 MR systems with Dedicated Computing ICNs. Effectivity: 1.5T & 3.0T Signa HDx 14M5A & 15.0 M3 MR systems with Dedicated Computing ICNs. Problem: It has been reported that one or more of the Dedicated Computing (DC) ICNs may have an obsolete Infiniband firmware version. This problem has caused symptoms after a software load or after a VRE configuration. This problem does NOT apply if all ICN computers have Infiniband firmware version 3.4.0 or greater. Symptoms: Scans stop or fail to deliver the expected number of images. The root cause is degraded Infiniband performance. Some sites may only see some protocols affected. In most cases, 3-plane localizers with multiple slices per plane generate errors. The following error message may be seen: Thu Jan 15 16:50:49 2009 2218537 1 MsaScanMgr.m 3038 Scan stopped, Scan Stopped due to error : SCT Interface Error. No source was detected for a SCT command Additional “ Infiniband backlog too large…scan stopped ” messages are seen as well in the APS window. 2. DEDICATED COMPUTING ICN HARD DISK FAILURE – Applicable to HDx 14M5A, and HDxT 15.0 M3 MR systems with Dedicated Computing ICNs. Effectivity: 1.5T & 3.0T Signa HDx 14M5A, and HDxT 15.0 M3 MR systems with Dedicated Computing ICNs. Problem: VRE configuration fails after software load. Symptoms: The ICN product configuration has two hard disks and is checked for validity at the time of VRE configuration of 14M5A or 15.0 M3 software. This check for valid product configuration did not exist in the earlier 14M5 software. The install fails with an “ unknown board type found in ICN system ” error. The VRE Install log file (/usr/local/bin/install/log/VRE_Install.log), will contain the following message if it did not detect the hard disks on the ICN: Unknown board type found in ICN system. DRIVES: Found, tconfig{DRIVES} drives 3. SUN ICNs NOT RECOGNIZED AT SYSTEM POWER-UP – Applicable to HDx 14M5A, HDxT 15.0, and MR750 20.0 MR systems with SUN ICNs. Effectivity: 1.5T & 3.0T Signa HDx 14M5A, HDxT 15.0, and MR750 20.0 MR systems with SUN ICNs. Problem: During system power-up, ICN(s) fail to boot up with the system. Symptoms: TPS reset will fail at power-up because the ICN(s) are NOT initialized properly. 4. 14M5A SERVICE PACK 3 PRE-SCAN FAILURES – Applicable to HDx 14M5A Service Pack 3 MR systems. Effectivity: 1.5T & 3.0T Signa HDx 14M5A Service Pack 3 MR systems. Problem: Pre-scan fails, TPS reset is required.During Autoshim pass of pre-scan, customer receives the syslog message: Sat Oct 4 11:32:20 2008 Error: 2255482 Host: b3 Process: LxMxConsumer File: base/MxServer.cpp Line: 836 Server Name: DPx Unknown server software error occurred. TPS reset may be required. Further errors, such as BAM Check Packet failed to reach the TPS , may follow. Symptoms: The problem is most frequently seen on the localizer (with autoshim) of a next exam, when the previous exam runs a large multiphase prescription, such as Vibrant with Multiphase variable delay (aka. Dynaplan). Other conditions are possible, but have not been reported.
