Symptom
Millennium CSE Detector PMTune Update not set the HV correctly and peak is extremely narrow. The peak is nowhere close to the vertical line of 140. Note the shape of the peak is not symmetrical - very distorted. As a result the system will start tuning with the peak set incorrectly.
