Symptom

Unable to Load Boot/Update CDROM when Loading IRIX 6.5 Operating System. Message is returned "unable to execute - unable to load dksc(1,3,8)sash64", and nothing else happens FE had just replaced the CD/DVD Drive. FE trying to reload 9.1 software and could not load the Boot/Update disk to load the IRIX 6.5 operating system. Error message is returned "unable to execute - unable to load dksc(1,3,8)sash64", and nothing else happens. Afterwards, as a test, we verified that we could mount a "patch" CDROM just to verify the CD-DVD Drive was working, by entering "mediad" with the "patch" CDROM in the drive. FE had just replaced the CD/DVD Drive. FE trying to reload 9.1 software and could not load the Boot/Update disk to load the IRIX 6.5 operating system. After entering the command "boot -f dksc(1,3,8)sash64 dksc(1,3,7)stand/fx.64 --x <Enter>", an error message is returned "unable to execute - unable to load dksc(1,3,8)sash64", and nothing else happens.
