The Head Holder was not in the Head Coil.

Symptom

Gradcal Tool Recommends a Drastic Change for Z-axis, and if Accepted, Images Look "Squashed" The InHouse engineer reports a problem occurring at two of his sites, each is an LX, one running 8.3 and one running 9.1 software. At each site, he is scanning the DQA-III in the Head coil, and using the DQA calibration tool to analyze the images and calculate gradcal. At each site, the X & Y axes converge properly, but the Z-axis is reported to be 143mm and recommends a drastic change. But he manually measures 100mm.
